APPLICANTS CERTIFICATION AND AGREEMENT

I certify that all information and statements made in this application are true, and complete. I further certify that I, the undersigned applicant, have personally completed this application. I understand that any untrue information, omission or misstatement of material facts on this application or any document used to secure employment may result in refusal of employment or, if hired, termination of employment, regardless of the time elapsed before discovery.
I hereby authorize my previous employers, educational institutions, and other references to release information regarding my employment, education, training, and qualifications to Armock Mechanical Contractors for purposes related to my suitability for employment. I understand that this authorization does not include consumer reports or investigative consumer reports obtained through third-party agencies, which will require separate written consent and notification under applicable law. I hereby release the company, my former employers and all other individuals, entities and associations providing such information from all claims, demands, or liabilities related to such disclosures, except as prohibited by law.
If hired, I acknowledge that my employment with Armock Mechanical Contractors Inc. is at will, and that the employment can be terminated by either the company or me at any time and for any reason, with or without cause or prior notice, unless expressly agreed otherwise in writing and signed by both me and the company president. I also understand that my employment will be subject to the company’s wages, benefits, rules, and regulations, which may change over time.
I agree to notify Armock Mechanical Contractors Inc. of the need for any accommodation within 182 days of becoming aware of the need or when it reasonably should have been known.
As a condition of employment or continued employment, unless otherwise provided for by law, I agree not to file any lawsuit or action relating to my employment more than 180 calendar days after the event and/or employment practice or action complained of including, but not limited to, employment termination and discrimination claims, claims for wages, salary, commissions, or expenses, and to waive any state or federal statutes of limitations to the contrary.
